Send the application form and evidence together with online application form stated in Topic 4 (scanned copies should have pixels not less than 300 dpi). |

Send a set of all evidence copies (with student’s signature) via post mail to Chulabhorn International College of Medicine, Thammasat University, Rangsit Campus, 99 Moo 18, Paholyothin Road, Khlong Nueng, Khlong Luang, Pathumthani, 12120, before 31 May 2019. If the copies were sent to the address later than the specified date, it will be considered that the registration was waived and the registration fee cannot be claimed back. |
